Fort Wayne, Ind. — The St. Joseph Community Health Foundation has awarded a $5,000 grant to Easterseals Arc of Northeast Indiana to help implement use of My25 meal planning tools.
My25 is a subscription program that provides customized menus, grocery lists and recipes designed to improve health of people with intellectual/developmental disabilities in group homes and other supported settings.
Easterseals Arc will use the grant funds to help support use of My25 in 10 group homes in Allen and Whitley counties. The My25 platform will provide menu plans customized to individual health needs along with grocery shopping lists, recipe instructions and nutrition details.
Through use of My25, Easterseals Arc will help individuals pursue goals for weight and body mass index, manage chronic health conditions, and reduce grocery costs.
This program is part of overall efforts by Easterseals Arc to improve health and wellness through emphasis on education and preventative health care.
