Fort Wayne, Ind.— The English, Bonter, Mitchell Foundation recently awarded a grant of $75,000 to Easterseals Arc of Northeast Indiana to support the purchase and implementation of a care management platform.
The care management system will be used throughout Easterseals Northern Indiana, the parent entity of Easterseals Arc and Cardinal Services. The new system will:
- Help the organization achieve quality measures by providing users with dashboards that include real-time scorecards and transparency into program effectiveness
- Support multiple value-based care initiatives
- Maximize productivity, workflow and decision-making with actionable information
As part of the care management system implementation, Easterseals will:
- Develop a data governance system
- Create cross-functional teams responsible for managing data and data-related processes
- Create a technical infrastructure for managing metadata
